Health in the Democratic Construction of Brazil



Abstract

The contribution of health to democracy must be recognized beyond the creation of the Brazilian Health System, encompassing its philosophical and political dimensions, which gave substance and vitality to democracy and were innovative in articulating relations between the State and society; its institutional architecture, which materialized a model of cooperative federalism; and its capacity to engage with the tensions between the instituting and the instituted, thereby continually renewing the Health Reform Movement. Material contingencies, a culture of domination and exclusion, and a weak balance of political forces imposed the reality of a restricted democracy, as well as limitations on the existing health system, distancing it from the model envisioned at the 8th National Health Conference. A new conference model created by the Frente pela Vida, with the realization of the 1st Free, Democratic, and Popular Conference in 2022, represents the expression of this instituting force—one capable of breaking even with one of its most cherished institutions, the 8th Conference, not in order to supersede it, but to revitalize it and to challenge enduring democratic limitations.
